ICHC offers services and programs that promote healthy, productive individuals while building stronger families and a healthier community.
Behavioral Health Services
Individual Counseling is a process through which patients work one-on-one with a behavioral health professional. It is tailored to the individual needs of the patient in a quiet and safe environment.
Marriage and Family Counseling assists individuals, couples, and families by offering guidance for mental and emotional issues within the family/marriage unit.
Couples Counseling helps couples of all types recognize, understand, and resolve conflict while improving their relationship.
Child and Adolescent Counseling is a therapy that focuses on children, teens, and adolescents. It helps the younger generation work through and deal with a large variety of challenges in life.
Group Counseling is a form of therapy within on a small group of people with similar concerns. They interact and explore each other’s issues while working out individual problems and concerns as a group. It offers support from not only the counselor but from the group members as well.
Our MAT program is a drug and alcohol outpatient program that combines medical and behavioral health support to provide comprehensive services to assist in the recovery from opioids and alcohol.
Clients in our program will receive individual and group counseling and medical care.
Clients in our program are provided with requirements that are designed to help them be successful in their recovery. Such as random urine screenings, group therapy, individual treatment planning and goal formation, and client contract agreements.
Telemedicine is a way to provide services from a distance using technology.
EMDR is used for post-traumatic stress disorder (PTSD) and Emotional Trauma. The purpose of EMDR is to reduce the anxiety and/or pain associated with past traumatic memories.
Our behavioral health department accepts private insurance, with the exception of Medicare and Medicaid, and offers sliding fee scale discounts and payment plan options. If you are on Medicaid and would like Behavioral Health Services, we offer a reduced rate of $10 a visit for those that qualify.

Please fill out this form if you are a new patient.
- Refill Request Form
Please fill out this form if you are requesting a prescription refill.
- Treatment of Consent for Minors
This form gives consent to another individual who is not the parent or guardian to approve care of a minor at ICHC. Examples include parents who will be out of town and the child stays with friends or relatives.
- Release of Information
This form authorizes ICHC or another entity to release health information to a specific organization or individual.
This form is needed for a Commercial Driver’s license physicals.
- School Entry Physical
Please fill out the School Entry form if your child will be entering school for the first time.
- Sports Physical
Please fill out the Student Health Review/Exam form if your child is needing a physical for school sports.
- Ladies First
